Please read these terms carefully before registering...

A. Attach 02 recent passport and 02 stamp size photographs and birth certificate of participant, 02 recent passport size and 02 stamp size photograph of the guardian with the filled in forms

B. The training sessions are ONLY designed by the participants aged 6 to 11 (Boys and Girls), participants aged 11-15 (Boys only) and 15-18 (Boys only)

C. No Make-up classes will be offered for any missed class

D. By registering this form, Parents/Guardians agrees that Bashundhara Kings Club may discontinue/cancel the participation of his/her participant at any given point for any kind of misconduct, subversive acts or disciplinary issues commenced by the participant
